The lost questionnaire

Welcome survivalists,

I didn't realize that the example questionnaire wasn't attached here. I added it in today. There is info available depending on tech level that the campaign is chosen to be. This is a recreation of the original one that was used when the farming town was originally used. This doubles as a way to give info, gather info about what the players want and it is a targeted version of a safety sheet. Depending on how the players answer should be how you modify the game to fit what they are okay with. It also has a question for a way to freak out a character. Hope this helps for you to craft your own.
